Photography / Video Release for Minor Child

This form lets you indicate whether you give permission for your student to be photographed or recorded during program activities for classroom, program, or promotional use.

This form is only required if you registered through a partner organization and not through the Reading Rookies website.

I hereby authorize Reading Rookies (“RR”) to take photographs and videos of me and my minor child or children in connection with RR activities. I attest that I am the parent or legal guardian of the child or children listed below and that I have full authority to consent to and authorize RR to use their likenesses and names. I further authorize RR, its assigns, and transferees to copyright, use, and publish such photographs and videos in print and/or electronic media for any lawful purpose. (The student's name will not be used.) I acknowledge that I have no expectation of privacy or confidentiality for myself or my minor child or children with respect to photographs or videos taken during RR activities. I also acknowledge that participation is voluntary and that neither I nor my minor child or children will receive compensation for RR’s use of any photographs or videos taken in connection with these activities.

I have read and fully understand the above important information, assumption of risk, insurance liability waiver and release of all claims and indemnification.
