A Quick Step to a Great Start

To ensure a safe and smooth experience for every student, we ask that all families complete the required forms before classes begin. These documents help us provide proper care, communication, and documentation throughout the program.

 

These forms are only required for students who did not register through the Reading Rookies website. For classes hosted by partner organizations (such as park districts or other locations), registration is often handled directly through that organization, and we may not receive all necessary information.

If you registered through a partner organization, please select and complete the forms below. Each button will take you to a separate page for that specific form. If you registered through the Reading Rookies website, these forms are already included in your registration and do not need to be submitted again.

Emergency Contact Form – Provides essential contact and medical information so we can quickly reach you if needed.

Photography/Video Release for Minor Child – Grants permission for photos or videos of your child to be used for program updates, promotional materials, or classroom documentation.
